Omnibus Swaption

I need to do a draft for a paper confirmation that is an Omnibus Swaption.
The only template that I have is the one I attached. Is it O.K. to use? Can I
use the current ISDA one and tweek it to be an Omnibus (ie: Determination
Periods, no reference to ISDA)?
